Job Summary:

The Palliative Care Manager (LMSW) is responsible for leading the development and launch of a new palliative care program, providing leadership and social work services to support patients with serious illness and their families. This role involves program design, staff supervision, compliance oversight, financial management, and collaboration with interdisciplinary teams to ensure high-quality, compassionate care aligned with regulatory standards and organizational mission.

Responsibilities:

• Lead all program startup activities including workflows, policies, and operational design.
• Manage program budget and financial performance.
• Supervise and support palliative care staff in collaboration with the Medical Director.
• Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards.
• Support provider credentialing and payer contracting processes.
• Ensure efficient scheduling, referrals, documentation, and care transitions.
• Drive program growth, customer service excellence, and continuous performance improvement.
• Deliver social work services during program start-up and as needed.
• Participate in the Manager On-Call Rotation for hospice services.

Required Skills & Certifications:

• Master's degree in social work.
• Experience with Electronic Medical Records and Microsoft Office products.
• Strong interpersonal, communication, and leadership skills.
• Demonstrated ability to collaborate effectively across disciplines and guide teams through growth and change.

Preferred Skills & Certifications:

• Hospice or palliative care related leadership experience.
• Prior program development experience.

Special Considerations:

• Participation in Manager On-Call Rotation for hospice services.
• Role involves launching and evolving a new program, requiring adaptability and innovation.

Scheduling:

• Full-time position with on-call responsibilities as part of manager rotation.
